Locum work in Queensland

Queensland is a two-speed locum market. Metro Brisbane plus the Gold Coast and Sunshine Coast spans Metro North, Metro South, Children's Health Queensland, Gold Coast HHS, and Sunshine Coast HHS. The regional and remote catchment is huge: Cairns and Hinterland, Townsville, Mackay, Central Queensland (Rockhampton), Wide Bay (Bundaberg), Darling Downs (Toowoomba), plus Far North Queensland communities and Torres and Cape HHS. The Medicare bulk-billing tripling shifted direct-billing economics across QLD clinics.

Payroll tax

Payroll-tax exposure in Queensland

The workplace’s exposure drives the service fee it pushes onto you. Sessional classifies it per workplace so the post-fee number is the one you price against.

Direct patient billing, GP shift, todayNo exposure

Queensland permanently exempts wages paid by a medical practice to a GP (contractor or employee) from payroll tax from 1 December 2024 (Public Ruling PTAQ014.1.2; Revenue Legislation Amendment Act 2024). The earlier amnesty ran to 30 November 2024. Non-GP contractor arrangements remain exposed under the relevant-contract test (PTAQ000.6.5).

How QLD rates actually land

QLD metro rates sit mid-band. Regional and remote rates are among the strongest in the country: Cairns, Townsville, Rockhampton, and Mackay are MMM 3 to 4 with active WIP eligibility, and FNQ runs MMM 5 to 7 with full rural loading. FIFO contracts into the Bowen Basin and Surat Basin pay whole-of-package.

Frequently asked questions

What happened with the QLD payroll tax amnesty?
The Queensland amnesty for historical contractor-GP arrangements ran to 30 November 2024. From 1 December 2024, wages paid by a medical practice to a GP (contractor or employee) are permanently exempt from payroll tax (Public Ruling PTAQ014.1.2). The relevant-contract provisions (Public Ruling PTAQ000.6.5) still apply to specialists, dentists, and allied health at the tiered 4.75 to 4.95 per cent rate, so non-GP direct-billing arrangements carry exposure and service fees are priced accordingly.
What credentials do I need to work as a locum in QLD?
AHPRA registration plus a Blue Card (Working with Children) issued by Blue Card Services when your role involves patients under 18. Doctors need S8 prescribing approval from Queensland Health Medicines Regulation and Quality for drug-dependent patients, and QScript real-time monitoring must be checked before prescribing monitored medicines. WorkCover Queensland runs workers compensation. Pharmacist vaccination scope in QLD includes patients 2 years and older, the lowest age floor in the country.
Is direct billing viable in QLD?
Direct patient billing is common across QLD GP clinics, and from 1 December 2024 GP wages are permanently exempt from payroll tax, so GP service fees no longer need to carry that cost (non-GP arrangements still do). The Medicare tripled bulk-billing incentives (introduced November 2023 and extended since) shifted the gross-billing baseline, making service-fee reconciliation the admin bottleneck. Sessional tracks gross billings, service fee, GST per line, and the payroll-tax exposure signal per workplace.
What rural incentives apply in QLD?
The WIP Doctor Stream covers MMM 3 to 7 across QLD regional centres and remote communities. Cairns, Townsville, Mackay, Rockhampton, Bundaberg, and Toowoomba are MMM 3 to 4; FNQ, the Torres Strait, and much of the state's west run MMM 5 to 7 with full rural loading. Sessional tags every shift with its postcode MMM so the loading on your payslip is verifiable against the agency or workplace contract.
How does the QLD pharmacy prescribing pilot affect pharmacist locums?
Queensland runs a full pharmacist prescribing pilot for select conditions. This expands scope beyond vaccination into limited S4 prescribing, which materially affects the rate and the service type you bill. Sessional supports pharmacist shift records with scope-specific rate tracking and flags when your workplace changes scope.
Which Hospital and Health Services does Sessional cover in QLD?
All of them. Metro North, Metro South, Children's Health Queensland, Gold Coast, Sunshine Coast, West Moreton, Wide Bay, Darling Downs, Central Queensland, Mackay, Townsville, Cairns and Hinterland, Torres and Cape, North West, South West, and Central West HHSs all sit as workplaces in one ledger.
